Services
Medicaid Waiver Case Management
We provide conflict free, person-centered case management services to individuals with intellectual and/or developmental disabilities, have an acquired brain injury, or have a physical disability.
We work with the Supports for Community Living (SCL), Michelle P. (MPW), Acquired Brain Injury (ABI), and Home & Community Based (HCB) Medicaid Waiver programs in Kentucky, including Participant Directed Services (PDS) for all waivers.
Applications for Medicaid Waiver Programs
Our team will assist you to apply for a Kentucky Medicaid Waiver Program, even if you do not already have Medicaid. We can help walk you through the application, approval, and enrollment process.
Resources
Our case managers can assist you to find the resources you need for you and your family's unique circumstances: Medicaid Waiver programs (which include community living supports, respite, adult day training, family home providers, behavior supports and residential services), Medicaid health insurance, Social Security insurance payments (SSI), and third party trust funds can all make a difference in your life.
